What is Primary Care?

Your primary care team includes family doctors, nurses and nurse practitioners. These healthcare providers build long-term relationships with you and your family, giving personalized care that considers your whole health picture and community needs.


What Is Team-Based Care?

Team-based care brings together different healthcare professionals who work together to take care of you. Instead of seeing one doctor, you benefit from the knowledge and skills of nurses, dietitians, pharmacists, and other healthcare workers who coordinate your care, through a referral from your family doctor.

No Family Doctor? We Can Help

Through our CarePoint Connects Program, we will match you with a nurse practitioner who can diagnose conditions, prescribe medications, and make specialist referrals. All services are covered by OHIP or IFH while we work on connecting you with a permanent family doctor.
