Pharmacist

Farah is a clinical pharmacist specializing in complex chronic care, with extensive experience in both community and hospital practice. She holds a Bachelor of Medical Science from Western University and a Bachelor of Science in Pharmacy from the University of Toronto.
She previously worked as a community pharmacist and designated manager at Shoppers Drug Mart, where she specialized in opioid addiction management (including opioid agonist therapy) and HIV medication management. She later transitioned to hospital practice as a clinical pharmacist at Toronto General Hospital.
In her hospital role, Farah provided specialized services for patients transitioning out of hospital, including medication education, counselling, and direct collaboration with healthcare teams to support a smooth and safe transition home. She also contributed to care within the infectious disease unit and assisted in the management of dialysis patients and individuals with complex chronic diseases.
Farah is a Certified Diabetes Educator (CDE) and holds additional certifications in TEACH-certified smoking cessation, the MINT Memory Clinic, and immunization and injection administration. She serves as an APPE/EPE preceptor for University of Toronto pharmacy students and has been recognized twice as Preceptor of the Year. She has also worked as a clinical assessor for pharmacy student practical laboratories.
Her clinical focus is on chronic disease management, including chronic kidney disease, heart failure, and diabetes. She is an active member of the Ontario Pharmacists Association.
