Marta Madzelan

Marta is a passionate primary care pharmacist with a wide range of experiences in delivering patient-centered medication management in collaborative primary care, academic and research settings.


 Her fascination in primary care began when working as a pharmacy student at both Credit Valley and Summerville Family Health Teams, focusing her research on chronic opioid use and quality improvement strategies. She has presented her research at both AFTHO and Mississauga Halton LHIN Learning Together Conferences and has been a recipient of the Bright Lights Award. As a result, she transitioned to the role of pharmacist at both family health teams, working collaboratively with various health professionals to provide comprehensive medication management of patient roster sizes of up to 70,000 patients. 


On her weekends, Marta enjoys working as a part-time clinical instructor for the International Pharmacy Graduate Program. Marta has special interests in chronic pain management and opioid use and comprehensive medication reviews that identify drug therapy problems and optimize patient outcomes. Her philosophy of practice is grounded in providing the best possible evidence-based care, in a collaborative and patient-centered approach. 


Marta completed her Doctor of Pharmacy and Bachelor of Science degrees from the University of Toronto. She continued to pursue her clinical focus in smoking cessation and palliative care, by completing her TEACH certified smoking cessation training and the LEAP Core course, respectively. She also enjoys being an active member of the Impact Primary Care Pharmacist Working Group. Marta speaks English and Polish.
