Sweta Patel

Sweta is an experienced and dedicated Primary Health Care Nurse Practitioner with over 15 years of nursing experience. She holds a Master of Science in Nursing and a Primary Care Nurse Practitioner Certification from McMaster University.


For the past seven years, Sweta has provided comprehensive primary care across the lifespan, with a focus on chronic disease management, preventive health, acute and episodic care, pediatric and women’s health—including pre- and post-natal care—mental health, and health promotion and education.


Sweta has a strong interest in program development and quality improvement (QI). She is a TEACH-certified smoking cessation counsellor, MINT Memory Clinic Certified, and a Preventive Care Practitioner through the BETTER Program. She also has advanced training in women’s health, offering family planning, contraceptive care—including IUD and implant insertions—and menopause management.


In addition, Sweta contributes to the Breathlessness Integrated Care Pathway, offering expertise in spirometry interpretation and management of COPD and CHF. She is bilingual in English and Gujarati, enhancing her ability to provide accessible and culturally competent care.
